Is It Your Back or Your SI Joint? How to Tell the Difference

Dr. Matt Rieder, DPT, doing hands-on hip and pelvis treatment with a patient on the table at Renew in St. Augustine

Does a deep ache flare up low on one side of your back every time you stand up from the couch? Or when you roll over in bed?

You've done the back exercises. You've stretched. Maybe you've had treatment aimed right at your spine. And it keeps coming back.

Here's something a lot of people never hear: it might not be your back at all. It might be your SI joint.

What is the SI joint?

Your sacroiliac joint, or SI joint, is where the base of your spine meets your pelvis. You have one on each side.

When that joint moves too much or too little, it gets irritated, and you feel it deep and low on one side. The trouble is, it looks a lot like a disc problem or a general back problem. So it gets treated like one, and it can go on for months without anyone looking at the real source.

The tell-tale signs

Every body is different, and I'll never diagnose you from a blog post. But here's what SI joint pain tends to look like.

It's on one side. The ache sits low, right around the dimple on one side of your back, or down in your buttock.

Getting up sets it off. Standing up from a chair or rolling over in bed are classic triggers.

It hurts to load one leg. Stairs. Getting out of the car. Standing on one foot to pull your pants on. Every time your weight shifts to that side, there it is.

It doesn't feel like a disc. Disc pain tends to sit in the middle of your back or shoot down the leg. SI joint pain is usually one-sided and low.

If that sounds like you, and the treatment aimed at your back never quite helped, that's a big clue. The trouble may be the joint where your pelvis meets your spine.

Why we don't guess

The worst thing you can do with one-sided low back pain is guess. If you treat the wrong thing, you lose weeks, sometimes months, and you start to wonder if you'll ever feel normal again.

So at Renew, we confirm it first. Your first visit starts with a full-body assessment, and like always, we start with your feet.

Why the feet for a back problem? Because everything in your body is connected. What happens in your feet affects what happens in your back, and vice versa. Your feet tell a story about how your whole body moves, and that story often explains why one side of your pelvis keeps getting overloaded.

By looking at the whole body, not just where it hurts, we find things that most clinics miss. We find out if it really is your SI joint, or something else, before we treat anything. And if it's something else, we'll tell you.

How we treat it

Once we know it's the SI joint, we calm it down with hands-on care. Then we go after the reason it keeps getting irritated.

That means building the strength and control around your pelvis so it stays stable. When your pelvis is stable, the SI joint stops flaring every time you shift your weight. And because we treat the whole body, we work on the other pieces too, like stiff hips, weak feet or a back that's been guarding for months. If you only treat the part that hurts, everything else that's off will bring the problem right back.

It's hands-on care and movement, not leaning on pain pills.

What about pregnancy?

SI joint pain during pregnancy is really common. The hormone and load changes of pregnancy can irritate the SI joints. Gentle hands-on care plus stability work is a safe way to settle it down.

You'll feel the difference on day one

Your first visit includes a treatment session. So you don't have to take my word that this approach works. You'll feel how your body responds that same day.

Every minute you're in the clinic, you're one-on-one with your Doctor of Physical Therapy. No techs. No group sessions. No doing exercises off in a corner by yourself.

Between visits, your exercises live in our app with videos, so you always know you're doing them right. You can message us anytime during the week, and Sandy checks in every week to keep you on track. Most people only need to come in about once every two to three weeks.

We don't stop when the pain stops

Getting rid of the ache is just the first step. Once your SI joint settles down, we move you into strength training. That's how you build a body that's strong and durable, so the stairs, getting out of the car and sleeping through the night stop being something you brace for.

You get your life back. The walks, the yard work, the yoga class, the round of golf. Whatever it is that makes you who you are.
